‘A Transition Booklet for Young Northlanders’ developed by CCS Disability Action and Tiaho Trust

Leaving school or home is a big step towards independence for anyone to take. For some, it can be exciting while for others, it can be full of confusion and uncertainty.

The purpose of this booklet is to provide disabled young people and their families/whanau in Northland with useful information that will support them as they “transition” from school and includes topics such as ‘Moving on from school’, Support Needs’, ‘Accommodation’, ‘Advocacy’, ‘Employment’.

To order a copy of this booklet please contact either CCS Disability Action or Tiaho Trust.
